A Message To Our Patients


For more information on MyChart, including how to set up and account and prepare for a TeleHealth appointment, please select from one of the available options on this page.

Note: A staff member will reach out to you before your scheduled appointment to confirm you are ready, and work with you if you have encountered any difficulties.

Username & Password Troubles

If you are having trouble remembering your username and/or password, MyChart offers self-reset options. Note: Most of the following information is similiar on a smartphone or computer, just laid out a little different visually. Any large differences are noted.

 
Username Trouble



(Smartphone): Open up the MyChart app, then click on "Forgot Password or Username?", and then click on "Forgot Username"



(Computer): Bring up the MyChart website and click on "Forgot Username?" on the right.


Enter in your first name, last name, the last 4 digits of your SSN, and your date of birth. Then hit Submit.


Next, you will receive a message stating that if the information you entered matches an account you will receive an email.


The email you receive will be similar to the one below, with your username listed.

Password Trouble



(Smartphone): Open up the MyChart app, then click on "Forgot Password or Username?", and then click on "Forgot Password"



(Computer): Bring up the MyChart website and click on "Forgot Password?" on the right.


Enter in your MyChart username, the last 4 digits of your SSN, and your date of birth. Then hit Next.


Next, you will receive a message statingthat additional verification is required. Scroll down to the bottom and hit "Send to my Email".


Open your email and locate the six character code (in this example it is 3XBBPT). Go back into MyChart and enter that code, or click the "Sign In to Jefferson Health" button in the email.


The email you receive will be similar to the one below, with the six character code (in this example it is 3XBBPT).


You will then be able to enter a new password (enter it twice).



You should now be able to log in with the new password.
